picking the polymorph?

How do you choose what you turn into? have polymorph but im stuck on spider,giant only. Lvl 8 currently. Perdus The Ham Thief

AHH !! Now I understand, go through the entire radial menu when right-clicking the quickslot.
Edited: Removed long lament on inability to right-click on anything on spellbook to change it.
