(1) When does the timer start? When you accept the mission, or when you click the door?
(2) What happens if you run out of time? i.e. do you lose favor with your contact, lose the ability to continue the storyline, etc?
Clock starts when u accept the mission I think and I have never failed one so I cannot comment.

Timed Door Missions-
Timer starts when you accept the mission
Timer does not stop for any reason (including logging off and server downs)
When you fail one (I had a server down in the middle of one), you get some xp/inf as a lesser reward... I can't comment on the storyline part, because I'm not sure.
it doesnt hurt the storyline,but there are some special rewards for door missions. i know at level 20 you can get a mission that has a chest with a SO enhancement in it,and you can get them at other levels to,or at least DO enhancements.
