I have a somewhat ancient 19" ADI Microscan that's recently ceased functioning properly. When first turned on and when switching resolutions/refresh rates, it takes anywhere from 30 seconds to 5 minutes to warm up. At first, it'll just be dark, but then the picture will graually fill in. Any idea what could be causing this? Or if it's easily fixable?

There's not much in a monitor that is cost effective to fix, especially with how cheap monitors are these days. I had a monitor that would do the same thing (but mine had an auto-off so I had to keep turning it on and off until it warmed up or it just wouldn't come on at all). Forget what it was, but it would cost about $150 for the new part...and I didn't pay that much for the monitor when it was new. After about 2 months it finally gave up and died.
