Do skills max at 200 or 250 and if you have a 200 or 250 skill + a mod will you make the thing every time if you have a (eg) 205 skill and trivial is 200?

Not exactly sure what you're asking, but I'll try to answer anyway. Since this is the Laboratory I'll assume you're asking about poisons.

Poison making skill is capped at (level + 1) * 5, until level 39 when it stops at 200. Then at each level above 50, you can gain five more points, till it caps at 250 at level 60.

Skill mod items effect your chance to succeed in making an item as though your skill level were (Your Skill Level) * (1 + Skillmod), so for example at 200 skill with a 5% mod item you would have a skill of (200) * (1 + .05) = 210 skill, for success vs failure check only (trivials not effected).

Trivial combines are not gaurenteed success, but they have a very high chance, roughly 95% of succeeding.

Dunno if PoP has raised the max skills from 250 (200 at level 40, if I remember it correctly).

On the other hand, I can verify that making stuff can be failed LOOOONG after it is trivial: Spider, Wasp and Solvent Gangrene (trivial at 20, 20 and 40) still give me an occasional failure with my Make Poison at 175. Go figure
